Family: Father - Bartemius Crouch Sr (murdered by Barty Jr) , Mother - Name Unknown (died in Azkaban). Suggested that the family was pureblood.
Description: Straw - coloured hair. Pale as a youth.
Known Associates: Lord Voldemort, Bellatrix Lestrange, Deatheaters, Winky the House Elf.
Deatheater: Was involved with Bellatrix in torturing the Longbottoms for information on Voldemort's orders and was caught and sent to the wizard prison, Azkaban.
Other notable facts: Bartemius Crouch Sr. had a grandfather who owned an Axminster, a twelve-seater flying carpet that the whole family could ride on. Mrs. Crouch was a short, wispy witch who had the terrible and sad experience of watching her husband renounce their son Barty and condemn him to Azkaban. - hp-lexicon
Barty escaped from Azkaban with the help of his mother, who was already dying. His mother and Barty took polyjuice potion switching places. His mother died in Azkaban in the form of her son and Barty was believed to be dead, the dementors buried his mother under Barty's name on the grave.
Barty Sr discovered his son was a supporter of Voldemort and so imprisoned him in his home under the imperious curse (hidden under an invisiblity cloak) and was looked after by Winky the house elf. Barty escaped fighting of the imperious curse and cast the dark mark as a warning for other Death Eaters who had gone free.
Barty helped return Voldemort to full power by taking the form of Mad Eye Moody in Harry Potter's 4th year at Hogwarts teaching Defence against the Dark Arts. From there he could manipulate the Triwizard Tournament so Harry would take part. Barty made the Triwizard cup a portkey leading the person to touch it to Voldemort.
It did lead Harry to Voldemort, but he escaped. Cedric Diggory had been killed by Wormtail because he was transported with Harry with the portkey. Barty Jr was going to kill Harry, but Dumbledore arrived in time to stun Barty and his polyjuice form of Mad Eye Moody disappeared and revealed his true identity. Barty told the whole tale to Dumbledore under Snape's strongest veriterserum.
Cornelius Fudge arrived at the scene and had Barty receive the Dementor's kiss on June 24th 1995.
